Introduction
We studied the CT images of 22 cases with malignant mesothelioma (MM), which were confirmed being pathologically and immuno-histo-chemically. International experts read the films individually, and we choose the typical cases. We agreed typical features about MM. Finally we made the guideline and reference images about MM. The selected features are Unilateral pleural effusion, Nodular pleural thickening, Interlobar fissure thickening, Mediastinal pleural thickening, Tumoral encasement of lung, Calcified plaque engulfment, Invasion, Diminished lung, Contracted hemithorax, Pleural mass, and others.

Methods
Three Thailand radiologists read the 22 subject CT films of mesothelioma (17 films were high probability) and non-mesothelioma (5 films ware low probability) independently, twice, before studying the guideline and reference films, and after studying them. We analyze the result about the features and probability.

Results
Sensitivity and Specificity of 1st time reading was 47.06% and 60% by Radiologist 1, 64.71% and 60% by Radiologist 2, 41.8% and 100%. Their crude agreement was 50%, 63.63%, and 54.55%. Sensitivity and Specificity of 2nd time reading was 76.7% and 40% by Radiologist 1, 88.24% and 60% by Radiologist 2, 58.82% and 100% by Radiologist 3. Their crude agreement was 68.18%, 68.18%, and 68.18%.

Discussion
2nd time crude agreement of all readers were better than 1st time results.
The reference CT images and guideline will help radiologists and other physicians identify mesothelioma.
