Methodology Case1; Phase1: A preliminary interview questionnaire. Phase 2: Seminars of health education to all workers and mangers (1229). Establishment of new accidents reporting and notification system. Training of the trainer Case 2; All employees and contractors were trained through 83 workshops in 2008 covering the following topic; Understanding your culture, Risk assessment, Improving for supervision, Driving for excellence, Rule of three, Working safety and Managing rule breading Case 3; six pillars program was implemented including: Awareness raising, Measurements of key unsafe behavior, Management commitment, Feedback, Root cause analysis and Ownership.30 training sessions were conducted involving staff and contractors. More than 2000 observations resaved in 2006 were recorded, trended and analyzed.
Results/discussion Case 1; incidence rate, frequency and severity rates have decreased from 227.8,94.9, 103.7 to 173.4, 72.2, 14.3 respectively. The direct return on investment after intervention program implementation was 1:5.02. Case 2; Bapetco staff achieved10.8 million man-hours LTI free on 16/4/2009, Bapetco contractors achieved 5 million man-hours LTI free on 1/2/2009, Human error incidents decreased from 90% to 77.6% through the last three years. Case 3; Rashptco received a Certificate of Achievement for implementation on effective work based BBS program.
